If you’re a CDL driver or motor carrier, here’s some good news during a confusing transition.
The FMCSA has re-issued a temporary waiver that allows drivers to continue using a paper copy of your medical examiner’s certificate as proof of medical qualification for up to 60 days after it’s issued.
This waiver is in effect October 13, 2025 through January 10, 2026. However, as of late January 2026, some reports indicate that the waiver framework has been extended further, often with new expiration dates like April 10, 2026, to accommodate remaining states yet to fully transition to the electronic system.
Why This Matters Right Now
FMCSA and the states are transitioning to a new system called National Registry II (NRII), which electronically sends your medical exam results directly to your state licensing agency.
While this system is the future, not every state and examiner is fully up to speed yet. That has caused delays that are outside a driver’s control, and FMCSA doesn’t want drivers sidelined because of paperwork timing.
What You Should Do as a Driver
- Keep a paper copy of your Medical Examiner’s Certificate (MCSA-5876) with you
- Make sure it’s current and issued within the last 60 days
- Don’t assume everything is updated electronically, carry the paper as backup
FMCSA is strongly encouraging medical examiners to continue giving drivers a paper certificate even while submitting results electronically.
What This Means for Motor Carriers
Motor carriers may also rely on a paper copy of the medical certificate for up to 60 days:
- As proof of qualification
- In the driver qualification file
- Without penalty during this transition period
This helps keep trucks moving while systems catch up.

This waiver does not change medical requirements.
Drivers must still:
- Be examined by a certified medical examiner
- Meet all physical qualification standards
- Be properly certified to operate
The waiver only addresses proof timing, not eligibility.
